The Surprise Doctor is a Circlejerk that had originated from the mid 2019 period of Dogelore. It is quite unknown how or when it started gaining popularity on the server, or how the general concept of "Surprise Doctor" even started.
The Original Footage of "Surprise Doctor" comes from a Spanish YouTube Channel, titled "Señor Wooly", owned by a white Spanish teacher that creates educational Spanish songs and videos for middle school and high school.
